STEP 1: Completely fill-out Onsite Payment Form (available at COUNTER 1)
* will accept CASH & CHECK payments ONLY*
STEP 2: Proceed to respective counter for payment
Counter 5: Radiology MD – MEMBER of CTMRISP and/or USP (PHP 9,000)
– Must be in good standing. Click MEMBERSHIP STATUS to check.
Counter 6: Radiology MD – NON-MEMBER of CTMRISP and USP (PHP 12,000)
Counter 7: Trainees, Radiologic Technologists, Allied Health Professionals and Students (prepare your hospital ID) (PHP 9,000)
STEP 3: Completely fill-out Onsite Online Registration Form below
– Prepare OFFICIAL RECIEPT to be uploaded as proof of payment
– Payment type: ONSITE
– INPUT OR number on the respective field
For any assistance , proceed to Counter 8
STEP 4: Await and present CONFIRMATION email to Counter 9 to claim your ID and kits
*Before proceeding to counter 9, please ensure you have the CONFIRMATION email to prevent any inconvenience*
